OPW 54039: Fall Arrest Phase I and OPW 54040: 36th and Maple Storm Sewer The project work will consist of the following: OPW 54039 - Fall Arrest Systems Phase I: Construction of fall arrest system footing and anchors at the following locations. 1. 1421 Ames Street 2. 3580 Maple Street 3. 6100 N. 24th Street 4. 6315 Florence Blvd 5. 6339 Pratt Street 6. 66th and Ames Ave 7. 68th and Evans 8. 69th and Taylor 9. 48th and Green, and 10. Sunshine Dr and Harrison Street 11. 72nd and Bedford. The Fall Arrest Systems shall be the DBI-SALA® SecuraSpanTM system as currently used by the City of Omaha. OPW 54040 – 36th and Maple Storm Sewer: Re-grade and stabilize the eroded areas on either side of the outfall structure A-1 with a flexamat, and modification of the top front screen, resetting retaining wall and decorative fence, and construction of 18 inches storm sewer. This project has an SEB reserve for ESB and SB Tier 1 and Tier 2 categories for 6% of the total bid amount.
