The Shreveport Job Corps Center is solciting cost proposals for a qualified optometrist to provide services to students ages 16-24. Services are: Provide eye exams for students with approval of Wellness Manager or designee Provide lenses, frames and contact lenses (with approved letter by Wellness Manager) for students. Services are to be provided for the period of November 1, 2009 to August 31, 2011 with a potential for three, one year options as agreed to by both parties. Cancellation: This agreement may be cancelled by either party hereto by giving a written notice to the other party thirty days prior to the effective date of cancellation. Proposal must contain a copy of current State of Louisiana license, resume, and proof of malpractice insurance. Job Corps is a national training and employment program administered by the Department of Labor to address the multiple barriers to employment faced by disadvantaged youth.
