OPTICAL SERVICES

Adams and Associates, Inc., contracting for the U.S. Department of Labor for the operation of the Gadsden Job Corps Center - a 225 student residential training facility located at 600 Valley Street, Gadsden AL is currently soliciting written bids for optical services.

All quotes should be submitted in writing to the Purchasing Department by Friday, June 7, 2013 by 3:00 p.m. Quotes received after the deadline and/or without all requested documentation will not be considered.

Vendor is to return written bid on company letterhead, vendor paperwork, and all licenses for doing business in the State of Alabama, all employee licenses required for performing work in the state of Alabama, Certificate of Liability insurance and list of references if no past experience with Center.

The period of performance is June 17, 2013 to September 30, 2013.

Submitted bid will show at minimum breakdown as follows:

• Cost per Comprehensive Eye Exam
• Cost per Metal or Plastic Frames
• Cost per Pair Plastic Lenses (Regular Prescription)
• Printed name and Signature of authorized person submitting bid

 If your quote deviates from specifications in any way, such exceptions must be noted on bid sheet. If unable to quote please indicate ‘No Bid' and return bid package to us.

Adams and Associates, Inc. reserves the right to reject any bids and select the bid deemed most responsible. Bidders should be aware of bid requirements for federally funded projects and act accordingly.

Bidders are placed on notice that this is a competitive bid process and cost is an important fact. Overall low bid does not constitute bid award.

Gadsden Job Corps Center is a federally funded facility and entitled to GSA pricing.

Payment terms are Net 30 days from date of invoice.

OPTICAL SERVICES

Vendor is to provide optical services the first and second Wednesday of the month between the hours of 9:00 a.m. and 11:00 a.m.

Vendor shall hold all licenses necessary to do business in the state of Alabama and provide copies of all licenses with bid package.

Vendor will see a maximum of 10 students per month and perform a comprehensive eye exam and fitting for glasses if needed.

Vendor shall provide laboratory services to fill prescriptions for glasses, replace broken lenses, as well as adjustment and repair of frames as required.

All eyeglasses must meet American National Standards Institute (ANSI) standards.

Vendor will provide Wellness Manager with a copy of the full examination to place in the student's file. Vendor will abide by all HIPPA policies.

Vendor is to provide a billing invoice that includes BPA number and shows individual lines for each item ordered.

Gadsden Job Corps Center does not accept backorders all orders will be considered complete upon receipt. Vendor must make Purchasing Agent aware of all shortages, via email ([email protected]) prior to shipping order.

 The vendor shall obtain and maintain insurance as follows if vendor's employee(s) or agent(s) will be on Center at time:

• Workers Compensation
• Comprehensive General Liability with Gadsden Job Corps Center listed as additional insured. ($1,000,000 per occurrence).
• Comprehensive Automobile Liability ($1,000,000/combined single limit)
• Fire Legal Liability ($500,000 per occurrence)
• The proposed shall cover any and all expenses not covered by said insurance including all deductibles.
