This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. The current solicitation number is 13-229-SOL-00021 and is issued as a Request for Quotation (RFQ). This acquisition is being conducted under Simplified Acquisitions Procedures in accordance with FAR part 13. The clauses and provisions referenced in this solicitation may be obtained in full text form at http://www.arnet.gov/far.

The Department of Health and Human Services/Indian Health Service has a requirement for an Optical Coherence Tomography System for the Bemidji Area Indian Health Service. Delivery/ship to location: the Cass Lake Indian Hospital, located in Cass Lake, MN. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-66. The associated North American Industrial Classification System (NAICS) code for this procurement is 423450 with a small business size standard of 100.00 employees.This requirement is a 100% Total Small Business set-aside and only qualified offerors may submit bid.


BASIS FOR AWARD

The Government intends to make an award without discussion therefore; the initial quotation shall contain the offeror's best price and shall be determined to be the overall best value and most advantageous to the Government.
