This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.

(ii) The Navajo Area Indian Health Service (IHS) intends to award a fixed-price commercial item contract in response to Request for Proposal (RFP) IHHC-K15-001.
(iii) The solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-16.
(iv) The associated North American Industry Classification System code is 423450. The requirement is Full & Open Competition.
(v) PRICE/COST SCHEDULE - Optical Coherence Tomographer, Required software, Printer, Table, Training, Delivery, Emergency Service Response.
(vi) PURPOSE OF THE CONTRACT: This RFP is for Ophthalmology Equipment Cirrus HD-OCT 5000 Version 7.5 [or equal,] to be delivered, set-up, and training provided for the Inscription House Health Center Optometry Department of Navajo Area Indian Health Services ( NAIHS. )
(vii) Date and Place of Delivery: 45 days after an Award. Inscription House Health Center, Highway 98 & Navajo Route 16, Tonalea, Arizona, 86044.
(ix) Evaluation Factors: Specifications, Warranty Terms, and Service Coverage.

CONTRACTING OFFICER AUTHORITY: Authority to negotiate changes in the terms, conditions or amounts cited in this contract is reserved for the Contracting Officer.
The Government will award a contract, resulting from this solicitation, to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered.

PROPOSAL SUBMISSION INSTRUCTIONS: All responsible offerors may submit a proposal, which shall be considered by the Agency.
