The Ohio National Guard is conducting market research and is seeking sources to provide various food/grocery products for the Ohio Army and Air National Guard throughout the State of Ohio.

B. Company Data Universal Numbering System (DUNS) number / CAGE Code

C. Company point of contact, mailing address, e-mail address, telephone number, fax number, and website address (if applicable).

D. Company point of contact, mailing address, e-mail address, telephone number of company point of contact that has authority and knowledge to clarify responses with the government.

E. Type of Company/Small Business Concern, ie: small business, woman-owned small business, veteran-owned small business, etc.

Interested parties are required to be registered and certified in the System for Award Management (SAM). Registration may be accomplished at https://www.sam.gov.
