1. This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued.

2. Solicitation number is N68908-18-Q-0017 and is being issued as a Request for Quotes (RFQ).

3.SHIPPING SPECIAL REQUIREMENTS:
Product is frozen on dry ice, it has to be shipped in a cooler with dry ice and delivered
within a specific timeframe (3 to 4 days) to avoid melting of dry ice that would
compromise the quality of the tissue
To ensure sterility of the allograft tissue, it must be sent in a cooler with dry ice. The
shipment must be received at our facility prior to the dry ice melting in order to keep theproduct safe for patient use
Order has to be shipped using FedEx International Priority method
Vendor must use FedEx Custom Critical Temp Control service to insure there is no
loss due to delays
