This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued.

The solicitation number is W81K0418Q0003 and is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-97 and Defense Federal Acquisition Regulation Supplement Publication Notice 20171228.

The North American Industrial Classification System (NAICS) is 339112, Surgical and Medical Instrument Manufacturing. The small business size standard is 1,000 employees. This procurement is a 100% Total Small Business Set-Aside.

DESCRIPTION OF REQUIREMENT: THIS IS A BRAND NAME ONLY REQUIREMENT. The Government has a requirement for the purchase and delivery of six (6) new Nidek ARK 1S auto refractor/keratometers. All equipment shall include a standard 12 month warranty and two (2) manufacturer's user manuals and/or instruction guides per instrument.

Electronic submission via e-mail is required. Only emailed quotes received directly from the quoter are acceptable. All quotes received after this due date and time may be considered late in accordance with FAR 52.212-1(f).
