THIS IS A REQUEST FOR INFORMATION (RFI) ONLY.

This is not a Request for Proposal (RFP) or Request for Quotation (RFQ).

In accordance with FAR 15.201, the Indian Health Service, Phoenix Indian Medical Center located at 4212 N. 16th Street, Phoenix, AZ 85016 is conducting a market survey to identify contractors the following: Neonatal Specialists (experienced Neonatologists or Neonatal Nurse Practitioners) services (Refer to attachment A for additional information.)

Interested contractors who meet the criteria above may submit information to the acquisitions department. This RFI is open from April 15, 2015 through April 24, 2014.

This RFI is for planning purposes only.

1. If your company can successfully perform the services
2. If your company is a Small or Large Business
3. Indicate if your company is registered in SAM
4. Your companies Data Universal Number (DUN)
