Dear Ladies and Gentlemen:

Reference: 13-247-SOL-00068 - The Phoenix Indian Medical Center (PIMC), Indian Health Service (IHS), has a requirement for Neonatal Specialist Services for the PIMC Pediatric Department.

You are invited to submit a Request for Quote (RFQ) to the referenced solicitation.

Standard Form (SF) 1449, Solicitation, Contract, Order for Commercial Items, requires complete Blocks 12, 17, 23, 24 and 30.

Offerors are required to enter price proposal quotes on Pages 7, Under Section B.5 Price.

Offerors are required to submit all required requested information in accordance to Section L.6, Business Proposal Instructions on Pages 40.

All sections must be completed otherwise the IHS will not consider your RFQ.

Prospective Contractors shall be registered and show that company record is in the System Award Management (SAM) website prior to the award of the contract. Contract awards cannot be made to offerors not registered or who do not have their company record in SAM. To comply with the mandate, the contractor must obtain a Data Universal Numbering System (DUNS) Number. The DUNS No. is a 9-digit number assigned by Dun and Bradstreet Inc., to identify unique business entities. If the offeror (Contractor) does not have a DUNS Number, it should contact Dun and Bradstreet directly to obtain one by calling 1-866-705-5711 or via the Internet at http://www.dnb.com. Refer to FAR Clause No. 52.204-6, Data Universal Numbering System (DUNS) Number (Apr 2008). If company record is not found in SAM prior to the award of the contract this shall automatically disqualify the offeror from further consideration.

It is the Offerors responsibility to periodically check for amendments made to the solicitation.
