The Navajo Area Indian Health Service contemplates issuing on a competitive basis, a firm fixed price purchase order for the purpose of Methamphetamine and Suicide Prevention Initiative (MSPI) technical assistance for continued implementation and administrative support for the six (6) Tribal, Federal and Urban MSPI programs. This support requires the services of a contract Local Evaluator. The Local Evaluator shall provide services in accordance with the attached Performance Work Statement (PWS) and provide deliverables as defined in Attachment A of the PWS.
This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) This solicitation is issued as Request for Quote (RFQ) No. NAV-15-0001.
The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Regulations. This solicitation is a "TOTAL small business set-aside" and will be conducted in accordance with FAR Part 12 and Part 13. The North American Industry Classification System code is 561990. The small business size standard is $10 million, though this solicitation is being competed as unrestricted.

Late offers - Any offer, modification, revision, or withdrawal of an offer received at the Government office designated in the solicitation after the exact time specified for receipt of offers is late and will not be considered. FAR CLAUSES: The provision at FAR 52.212-3 Offeror Representations and Certifications Commercial Items. Offerors shall be registered in the System for Award Management (SAM) at www.sam.gov. Also in accordance with the clause, all prospective Offerors must be actively registered in the System for Award Management prior to award.
