This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued.

The Centers for Disease Control and Prevention (CDC), National Institute of Occupational Safety and Health, has a requirement for Medical Services at the NIOSH Morgantown, WV location.

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This combined synopsis/solicitation is being issued as a Request for Quote (RFQ) under solicitation number 75D301-18-Q-69353 using FAR Subpart 13, Simplified Acquisition Procedures. Any incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-100, effective August 22, 2018.

This requirement is solicited as a total Small Business Set-aside under NAICS code 621999 - All Other Miscellaneous Ambulatory Health Care Services with a small business size standard of $15 Million.

The CDC intends to award a single firm fixed priced IDIQ contract with a base and four (4) option years. The government reserves the right to award any, all, or none of the options under this requirement.

The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition and a statement regarding any addenda to the provision.

This award will be made to the offer under the conditions found in FAR 52.212-2 Evaluation of Commercial Items.

A Firm-Fixed Price IDIQ contract will be issued in writing to the successful offeror.
