CITY OF MONMOUTH MONMOUTH AVE N AND JACKSON ST W SIDEWALKS Bids due 2:00 pm, January 28, 2021 ADVERTISEMENT FOR BIDS Sealed bids for the furnishing of all labor, materials and equipment necessary to complete the "Monmouth Ave N and Jackson St W Sidewalks" will be received at the office of Russ Cooper, City of Monmouth Public Works Director, 401 Hogan Road, Monmouth, Oregon, 97361 until January 28, 2021 at 2:00 pm, (PDST). The bid opening will follow immediately thereafter. Proposals shall be clearly marked with the project title, showing date and time of bid opening, and with the name and address of the bidder. All proposals received will be publicly opened and read aloud at the above place, time and date. Bids received after the above date and time will not be accepted. Bids must be written on the forms provided in the Contract Documents and must be signed by the Bidder, or an authorized representative. A non-mandatory pre-bid meeting will be held January 22, 2021 at 2:00 at the project location listed below. This project consists of the following: SCHEDULE A: Monmouth Ave. N. • Remove 237 LF of existing 5' sidewalk • Place gravel base for new 8' wide side walk to COM min. depth • Install 237 LF of new 8' wide sidewalk • Remove and replace 2 existing commercial thickness driveway approaches • Tie into existing 6' sidewalk @ Jackson • At existing electrical vault in sidewalk - add 3' wide by 5' long sidewalk to match new alignment SCHEDULE B: Jackson St. W. • Remove 324 LF of existing curb and sidewalk • Place gravel base for new 8' wide side walk and curb to COM min. depth • Install new curb and gutter for ADA ramp and 324 LF of street frontage • Install 324 LF of new 8' wide sidewalk • Remove and replace 2 existing commercial thickness driveway approaches • Tie into existing 6' sidewalk @ Monmouth Ave N • Install new "bulb out" ADA ramp at southeast corner of Warren St. N and Jackson St. W. - north line of bulb-out shall line up with curb on east side of intersection north of Main Street Park. • No monolithic concrete pouring will be allowed - curb and sidewalk will be required to be poured separately. • Abandon existing catch basin at SW corner of Jackson and Warren and install tee • Install 2-Type 2 catch basins on each side of new ADA bulb-out ramp. The catch basin in Jackson St W is to be installed on existing storm line with pipe cut after installation. The catch basin on the Warren St side of the ramp shall be connected to the tee in Jackson St W with 10" PVC 3034. All construction shall be completed by April 2, 2021. This includes construction and any final correction items upon City of Monmouth final inspection. At a minimum, the following plan room services will receive sets of Contract Documents for the work contemplated herein: Oregon DJC Plan Center, Portland OR; Salem Contractors' Exchange, Salem OR; Each proposal shall be accompanied by an unconditional certified check, cash ier's check or bidder's bond in the amount of five (5) percent of the total amount of the bid, payable to City of Monmouth, Oregon, as a guarantee that the bidder, if his/her proposal is accepted, will promptly execute the contract, secure payment of worker's compensation insur ance, and furnish a Separate 100% Corporate Surety Performance Bond and a Separate 100% Payment Bond for the faithful performance of the contract. The contractor may be asked to have their Surety furnish a letter certifying they have currently reviewed the Contractor's finan cial statement and that the Contractor is financially sound prior to award. Qualifications: Bidders must be an established business (minimum three years) doing like projects and be registered with the Oregon Construction Contractor's Board. Prime bidders must be prequalified under the provisions of ORS 279C.435. Approved qualification statements must be submitted to Monmouth Public Works along with contractor's bid. References: Prime Bidders must present three (3) references, including business name, contact name and title, phone number and email address, for projects similar in size and scope and completed within the past five (5) years. References must be included in the bid packet. Failure to supply references will result in bid rejection. A subcontractor listing is required and can be submitted with the bid. If the subcontractor listing is not submitted with the bid, it must be received within two (2) hours after the bid closing time and date and can be emailed to dkarr@ci. monmouth.or.us, fax 503-838-0201, or hand delivered to 401 Hogan Road, Monmouth, Oregon 97361. Failure to supply a correct subcontractor listing may result in bid rejection. The Owner reserves the right to reject any Contractor who has not performed acceptably on past projects with the Owner, or others (this includes sub-contractor relationships). The Owner reserves the right to reject any Contractor who the Owner determines cannot do the project within the timeline or does not have the resources and financial stability to complete the project. The Owner reserves the right to postpone award for a period not to exceed 14 days after the bid opening; to reject any or all bids; to reject any bid not in compliance with prescribed solicitation procedures and requirements and other applicable laws; to waive any informality in a bid; to increase or decrease bid items depending on available funds; and to make awards in the best interest of the Owner.
