REQUEST FOR PROPOSALS (RFP) SAN MIGUEL COUNTY Miramonte Building 3rd Floor Windows Replacement INTRODUCTION San Miguel County, Colorado (“County”) is seeking proposals from professional companies or individuals capable of providing contractor services for window removal and installation of new windows purchased by the County. Interested professional organizations or individuals are requested to submit statements of experience, qualifications and costs to the County. The project award is contingent upon approval by the San Miguel County Manager. Firms or individuals will be ranked on experience, cost, and qualifications, including but not limited to previous, similar projects, key personnel qualifications, and availability. To finalize San Miguel County’s energy efficiency project we are seeking a contractor to safely remove and replace nine, 3rd-floor windows in the front and back of the Miramonte Building at 333 West Colorado, Telluride, CO. The new windows will be on site for the project. The County has secured all necessary permits for this project. SCOPE OF WORK The scope of work includes the following: Project Time Frame- We will need full access to the San Miguel County ballot box April 17th through May 3rd. The ballot box is located in the breezeway/stairwell, the front right side of the building and cannot be blocked during that time frame. Ideally, the front window work would take place during Telluride's shoulders eason between April 3-16 or May 4-27th. This work will take place above businesses that remain open during the off-season. We would like these businesses to remain open as much as possible with intermittent closures. It is important that Sunshine Pharmacy remain open throughout the project, as well as the ADA Accessible entryway to the Building. If this time frame cannot be met, we would like to discuss other dates. Please review the Telluride Building Code requirements for hours and days of operation. Five windows facing Colorado Ave. Rear Windows- Vinyl Window - 96.75"x71" RO - 3 4.00 EA wide fixed middle single hung on either side. Four windows on the backside of the building facing the alley and employee parking. Dumpster- Many of the windows will be taken by county employees, but the Contractor will need to secure a dumpster to dispose of construction debris. Labor- Remove and install- Windows will be a retrofit install and will be ordered without fins, installed into openings and caulked in place, leaving trim and drywall in place during installation. Please include an estimate for trim replacement. Trim out, new extension jambs, drywall and paint touch-ups. Allowance for a touch-up to drywall and paint. This allowance is based on minor touch-ups to all windows if needed. Flash, and concrete fill openings and gaps. Fill existing gaps in mortar and windows with concrete and metal flash and install peel-and-stick weatherproofing. This does not cover any framing damage that may be present. Lifting equipment or scaffolding to assist in removal, installation and repair/painting. Coordination with County staff for building interior work. The third floor is office space for County staff and San Miguel Public Health Clinic. The County will need advanced notice of interior work. [email protected]. Submittal shall have the subject line: REQUEST FOR PROPOSAL: Miramonte 3rd Floor Window Replacement, and received by April 14th, 2023, at 5 pm to the Natural Resources Director. Any submittals received after this date and time will be deemed unresponsive. The County reserves the right to accept or reject any and all proposals, to re-solicit proposals, to waive any irregularities, to negotiate pricing and to select the proposal deemed to be in the best interests of the County. Issuance of this RFP does not obligate the County to award a contract nor is the County liable for any costs incurred by the proponent in the preparation and submission of the proposal. FEE SCHEDULE The Contractor shall provide a Fee Schedule table showing fully burdened labor rates for all labor categories of employees envisioned to provide services that align with the work, including transportation costs, if applicable. Billing will be on a monthly basis for any services provided. Professionalism and Compliance with Laws: The selected vendor must perform the work in a professional manner in accordance with all applicable County, State, and Federal laws and regulations. INVOICING All invoices shall be itemized with costs separated by materials, labor hours and rates, equipment rentals, miscellaneous services, etc. and shall reference the location and unit #. [email protected]. INSURANCE It is desirable that respondents carry insurance in the approximate amounts set forth below. Respondents who carry lower limits should state so in the proposal. Commercial General Liability Insurance: Comprehensive general liability insurance insuring against any liability for personal injury, bodily injury or death arising out of the performance of the Services with at least Four Hundred and Twenty Four Thousand ($424,000) for a single incident and One Million Dollars and Ninety Five Thousands Dollars ($1,195,000) general aggregate. Comprehensive Automobile Liability Insurance: Comprehensive automobile liability insurance insuring against any liability for personal injury, bodily injury or death arising out of the use of motor vehicles and covering operations on or off the site of all motor vehicles controlled by Contractor that are used in connection with performance of the Services, whether the motor vehicles are owned, non-owned or hired, with a combined single limit of at least One Million One Hundred and Ninety-Five Thousand Dollars ($1,195,000). Professional Liability Insurance: Errors and omissions professional liability insurance insuring Contractor against any professional liability with a limit of at least Four Hundred and Twenty Four Thousand ($424,000) for a single incident and One Million Dollars and Ninety Five Thousands Dollars ($1,195,000) general aggregate. Other Insurance: Worker’s compensation insurance (unless Contractor provides a completed Declaration of Independent Contractor Status Form) and other insurance required by applicable law.
