Background:
Mashpee Service Unit located at; 483 Great Neck Road South, Mashpee, MA 02649, is an Indian Health outpatient Medical / Dental / Behavioral Health facility. This facility is comprised of 3 buildings; a medical office building, a dental office building, and an administrative office building. All buildings were purchased between 2011 and 2013. To coordinate with the Tribal construction of the Mashpee Wampanoag Governmental Center building, the buildings were relocated to their current site at the end of 2013.

All utilities serving the buildings are located underground and include Telecommunications, Natural Gas, Electric, Water, and onsite sewage system operated by the Tribe. Utility locations are not marked at the site currently.

During the relocation all site work was completed with the final asphalt coat being completed Spring of 2014. It is known that unsuitable soils were removed from the site and new soils brought in to accommodate the existing asphalt and gravel parking areas. Overall site soils are sandy.


Work contemplated:

1. Asphalt pavement parking area to be added at existing gravel parking area. Size: 60 foot by 100 foot. Parking area shall be stripped and concrete parking stops are to be provided for 22 (90 degree) parking spots (9'x18'). Excess gravel is to remain on site and can be used as a base, if suitable for concrete work in this project. Asphalt to match existing, see Bituminous Concrete Pavement detail below.

2. Concrete trash dumpster pad:
20 foot by 20 foot; 6 inches thick; Utilize rebar mat throughout slab. Minimum of 3500 PSI concrete. Broomed finish texture. Provide proper base. Add control joints added to make 4 sections (10 foot by 10 foot)

3. Sidewalk:
60 Linear feet of sidewalk 5 foot wide. Minimum of 4 inches thick. Provide proper base. Sidewalk will be installed from the existing sidewalk between the Dental and Admin buildings to provide access to Dental rear stairs and the Picnic Table Pad. Use #10 welded wire centered vertically in slab. Finish to match existing sidewalks NOTE: Underground utilities known to exist in the area of this sidewalk


4. Picnic Table Pad:
12 foot by 16 foot pad for picnic table. Minimum of 4 inches thick. Finish to match existing sidewalks. Control joints added to make a minimum of 4 sections (6 foot by 8 foot) Use #10 welded wire centered vertically in slab
