Licensed Pharmacist 
State of Work:  The contractor shall participate in committee activities, special projects.  Perform ordering, receiving, dispensing, storing pharmaceuticals, including controlled substances, association with on going clinical and research protocols.  Provide periodic inventories and reporting to Clinical Research Center Manager.  Maintain pharmaceuticals in crash carts and satellite facilities, performing administration tasks associated with registered pharmacy as required by New York State, FD, and DEA.  Provide technical assistance to administration technical and medical staff responsible for conduct of clinical research using controlled substances to ensure protection of human subjects for controlled substances.  Available reimbursement of out of pocket expenses for extraordinary travel (50 miles or more) involving personal auto use, compendia, formulary services necessary to operate the pharmacy efficiently.

The contractor must have New York State license, must provide itemized invoice for professional services.  Must provide at the end of each calendar month all hours/days worked.

This requirement will require a physical by a Occupational Health Physician.
