Amendment 02

This amendment changes and clarifies the original BAA. Amended regions are
highlighted in turquoise.

1. Section 4.B (Proposal Content Specifics, Volume 1 - Technical & Management Proposal) is amended to add "7. -Draft Human Subjects Protocol".

2. Section 4.B.1 (Volume 1, Technical and Management Proposal, 4.B.1.4 Section 4) is amended to add "Attachment 6: Consultant Commitment Letters" and move "Draft Human Subjects Protocol. Template provided in Appendix H." to Attachment 7.

3. Appendix A (Academic Institution Acknowledgement Letter Template) is amended to add "Attachment 1 to Proposal Volume 1" for clarification purposes.

4. Appendix D (Template for Organizational Conflicts of Interest Certification Letter) is amended to add "Attachment 3 to Proposal Volume 1" for clarification purposes.

5. Appendix G (Templates for Restrictions on Intellectual Property Rights) is amended to add "Attachment 2 to Proposal Volume 1" for clarification purposes.

6. Appendix H (Sample Cover Sheet and Table of Contents for Draft Human Subjects Protocol (Required)) is amended to add "Attachment 7 to Proposal Volume 1" for clarification purposes.
