This is a request for quotes for kennel AND veterinary services for a possible total of five years. The contemplated contract will be for one base year and four option years. Please provide a price quote form for each of the five years. If you wish to quote the same price for all five years, please clearly and legibly state this on your price quote form. Contract years will run for 12 months after date of award of contract, most likely starting 1 JAN 09. The Service Contract Act [52.222-41] and Department of Labor Wage Determination 05-2495 (Rev. -9) apply to this order and are hereby incorporated by reference. Please complete the SIGNED price quote form attached, and return with this form. YOU MUST ALSO COMPLETE AND SIGN Blocks 12, 13, 14, 15 and 16 on page one of this form. Quotes that are incomplete, unsigned, or late will not be considered. All quotes must be RECEIVED by the Contract Specialist no later than 10 DEC 08, 3:00 P.M. Eastern Standard Time (Indianapolis). The cost of driving to and from your facility and the canine work site will be added to your quoted prices in the evaluation of the total cost of your proposal.
