The serial number of the unit is EM336526. In addition, the contractor shall install the CPU board and ensure the functionality of the Innova 3100. This shall include any additional troubleshooting after the CPU board is successfully installed. Place of Performance: Central Arkansas VA Medical Center 4300 W. 7th St, Little Rock, AR 72205 Background: The GE Healthcare Innova 3100 was installed in November 2009. Its serial number is EM336526 and its VA# is MX1096301. Personnel are unable to operate the Innova 3100. It is locked out and providing an error message Xray Not Available and Database Inconsistency . GE Healthcare technician provided initial troubleshooting and stated the CPU board is causing the error message. This is an emergency request. We have one additional invasive cath lab. If this cath lab becomes inoperable, we will be unable to see patients. Performance period: Within 1 month of obtaining all necessary parts.
