This solicitation is a TOTAL SMALL BUSSINESS SET-ASIDE for commercial items prepared in accordance with the subparts in Federal Acquisition Regulation 12, 13 and 15 and Veterans Administration Acquisition Regulations as supplemented with additional information included in this notice. Solicitation number RFQ 673-156-06 applies and is issued as a request for quotations. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-12. The North American Industry Classification Systems (NAICS) code is 811219, size standard $6.5 million. Provide preventive maintenance and repair services for three (3) HDI and ATL (Philips) HDI 5000 Ultrasound systems located at the James A. Haley Veterans hospital, 13000 Bruce B. Downs Blvd., Tampa, Florida 33612. Period of contract shall be from October 1, 2006 through September 30, 2007 with four optional years.
