Statement of Work
Title Service on station to repair the GE Innova 3100 X-Ray System.
Scope Vendor will make all repairs required to return the GE Innova 3100 back to normal operating condition. All travel, labor, and parts required to complete the task are to be provided by the vendor. The Innova 3100 scanner is located at the John L. McClellan VA Hospital in Little Rock, AR. Room # is 5C135.
Background The Innova 3100 has been installed at the VAMC in Little Rock and in use since November 2009. The system has not been under any type of service/maintenance agreement within the last 12 months. The system is currently down and will not boot up properly.
Performance Period Vendor is to begin repairs as soon as purchase order is issued. Repairs are not expected to require more than two 8 hour days to complete.

Security:
Information Systems Officer, Information Protection:ÃÂ The contractor will not have access to VA Desktop computers nor will they have access to online resources belonging to the government while conducting services.ÃÂ
Privacy Officer:ÃÂ
The contractor will have access to Patient Health Information (PHI) and they will have the capability of accessing patient information during the services provided to the VA.
Records Manager:ÃÂ
There will not be federal records created, maintained, used or disposed of with this contract;ÃÂ regardless of format [paper, electronic, etc.] or mode of transmission [e-mail, fax, etc.].
