Amendment 3 is issued to respond to questions af follows:

1. Do you require samples for all 3 colors listed for item #1? Yes.

2. Are all three colors in use, or are those just colors of option? All 3 colors are in use.

3. I have one question regarding the isolation gown RFP, do we have on all 3 schedules or can we bid on just two? Vendors may quote on all items or 1 or 2 items.  Quotes must include samples and other information required by the RFQ in order to be considered.

The RFQ closing date of June 5, 2013, 4:00 p.m. Central Standard Time is not changed.
