The Environmental Health Investigations Branch is looking for a Contractor, in the San Francisco Bay Area region, who has a history of serving Black communities, and experience providing asthma self-management and environmental trigger education. The services shall be performed virtually or in-person with organizations (e.g., federally qualified health centers, Medi-Cal managed care plans, and community-based organizations) serving primarily Black communities, who have the poorest asthma outcomes of any racial/ethnic group in California. The Asthma Management Academy (Academy) is a series of educational modules delivered to educators who primarily provide (or will provide) culturally and linguistically accessible, comprehensive asthma self-management and healthy homes education to patients or clients. Bids must address all of the services described in the Exhibit A entitled, ¿Scope of Work¿. Contractor agrees to provide asthma self-management and environmental trigger education and will be responsible for carrying out activities and documenting deliverables to achieve the agreement goals. All Proposers. Including out-of-state firms must comply with California¿s DVBE participation requirements. Proposals without a minimum 3-percent participation from a DVBE Certified prime or subcontractor will be deemed non-responsive.
