The Bureau of the Public Debt, on behalf of the Armed Forces Retirement Home (AFRH), intends to award on a sole source basis to Commission on Accreditation of Rehabilitation Facilities (CARF), 4891 E Grant Rd, Tucson AZ 85712-2704 to conduct an accreditation process for the Washington and Gulfport Campuses.

The purpose of this award is to procure survey services for the facilities in Washington and Gulport and assess the standards of care in order to determine accreditation status. CARF will perform a Continuing Care Retirement Community (CCRC) accreditation survey and perform an accreditation survey for Dementia and the nursing facility for the Washington Campus; perform a CCRC accreditation survey for the Gulfport Campus.

This notice is being posted in accordance with FAR 5.2 and is not a request for proposals. The Bureau of the Public Debt will consider written responses received no later than 2:00 p.m. ET on Wednesday, May 25, 2011. Responses must include sufficient evidence that clearly shows that the vendor is capable of providing the requested survey. If no written response is received that clearly demonstrates an ability to meet all requirements, is more advantageous to the Government, and is received by the aforementioned deadline, BPD shall make award on a sole source basis to CARF.
