This is a combined synopsis/solicitation for noncommercial items prepared in accordance with the format in Federal Acquisition Requirements (FAR) Subpart 12, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation will not be issued.

The solicitation number for this requirement is 75D301-18-R-67833 and is hereby issued as a Request for Proposal (RFP).

The Centers for Disease Control and Prevention intends to award a firm-fixed-price contract for the following requirement, please either fill in the dollar amounts or provide a separate excel spreadsheet containing the same information including a list of line item number(s) and items, quantities, units of measure, and options, if applicable:

Your quote shall include the following:

Solicitation, contract, or modification number;
Name and address of offeror;
Name and telephone number of point of contact;
Name, address, and telephone number of Cognizant Contract Administration Office;
Name, address, and telephone number of Cognizant Audit Office;
Proposed cost, profit or fee (as applicable) per year and total for all years, see Statement of Work
Capabilities in accordance with the Statement of Work

The quote format is at the discretion of the offeror.
It is the offeror's responsibility to be familiar with the applicable clauses and provisions. Clauses and provisions may be accessed via the Internet at website http://farsite.hill.af.mil.

This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-82 (effective 5/7/2015)
This acquisition is set aside for small business under North American Industry Classification System (NAICS) code 112930 and small business size standard of $.75 million. A Firm-Fixed Price contract will be issued in writing to the successful offeror.
