Contractor shall furnish all labor, equipment, tools, parts, materials, supplies, traffic control, travel, and incidentals necessary to provide Encampment Site Waste Removal and Cleanup Services within the city limits of the City of Berkley, the City of Emeryville and/or the City of Oakland in Alameda County to the California Department of Transportation (Caltrans) on an on-call, as-needed, and/or urgent basis to Caltrans, as described in this Agreement. Bidder shall possess at the time of bid submittal and for the duration of the Agreement, a valid and current registration with the Department of Industrial Relations (DIR). Caltrans will verify bidder¿s DIR registration, it is not necessary to provide a copy. Bidder shall possess at the time of bid submittal and for the duration of the Agreement, a valid and current Motor Carrier Permit issued from the California Department of Motor Vehicles (DMV) as described in Item 9 of this Invitation for Bid and in Exhibit E of the Proposed Forms of Agreement, Attachment 10. Caltrans will verify bidder¿s Motor Carrier Permit, please provide a copy. The prime Contractor shall maintain the following trainings, permits and licenses for the duration of this Agreement: Hazardous Substance Removal (HAZ) Certification from California Contractor¿s State License Board (CSLB). Bidder must have at time of bid submittal and for the duration of the contract, a valid, current license issued by the CSLB, for the type of work to be performed. Bidder shall obtain, pay for, and maintain, in good standing, all necessary licenses and permits to accomplish the work. Bidders will be considered non-responsive unless proper licensing requirements are met. An invalid license will result in rejection of the bid. Positive verification of a valid license issued by the CSLB will be performed by Caltrans (Bus. and Prof. Code Section 7028.15). Hazardous Materials Certificate of Registration issued by U.S. Department of Transportation Pipeline and Hazardous Materials Safety Administration. Hazardous Materials Transportation License issued by CHP. Trauma Scene Waste Management Practitioner Certificate from State of California Department of Public Health, Medical Waste Management Program. Waste Tire Hauler Registration Decal for each vehicle with effective and expiration dates issued by Cal Recycle. Hazardous Waste Transporter Registration issued by the California Department of Toxic Substances Control. Confined Space Entry Certification, from each employee as required by the Department of Industrial Relations, Division of Occupational Safety and Health (Cal/OSHA). A permit and contractors program documents will also be required prior to the start of work. Hazardous Waste Operations and Emergency Response (HAZWOPER) Certificate of initial 40 hours of training and annual 8 Hour Refresher training from each employee, prior to the start of work. The prime Contractor or subcontractor shall maintain the following trainings, permits and licenses for the duration of this Agreement: Contractor or subcontractor applying, or spraying disinfectant shall possess a Pest Control Business (Branch (PCB) or Main (PCM) license and have a Qualified Applicator License (QAL) with Category K (health related) staff to oversee the business operations. Both of these licenses are issued by Department of Pesticide Regulation (https://www.cdpr.ca.gov). The PCB/PCM is required at the time of bid. The QAL will be verified at the start of work. Contractor or subcontractor applying, or spraying disinfectant shall also be registered with each individual county agricultural commission where the disinfectant will be applied. Proof of this requirement shall be provided to Caltrans Contract Manager prior to applying or spraying disinfectant.
