This is a PRESOLICITATION NOTICE for Solicitation 75H70123R00040 (Polacca, AZ 86042) a construction project prepared in accordance with FAR Part 36.

SCOPE OF WORK: The purpose of this project is for the completion of additional residential quarter units for the staff members for the Hopi Health Care Center (HHCC).
     - The proposed project will construct four new staff quarters buildings, each with four one bedroom units, for a total of sixteen (16) new residences.
     - The work consists of utility infrastructure, pavement, a lift station, the units, and the furnishing of each unit.

LOCATION OF PROJECT:  The project is located at HHCC, Highway 264 Mile Post 264, Polacca, AZ, 86042

PERIOD OF PERFORMANCE:  248 days

CONSTRUCTION MAGNITUDE:  In accordance with FAR 36.204, Disclosure of the Magnitude of Construction Projects, the anticipated project magnitude is between $5,000,000 and $10,000,000.

SITE VISIT: Details regarding the Site Visit are included in the solicitation. 

CONTRACT TYPE: The Indian Health Service intends to award a single Firm Fixed Price (FFP) contract in support of this requirement. This solicitation will be procured as a Request for Proposal (RFP) in accordance with FAR Part 36 procedures with Lowest Price Technically Acceptable (LPTA) evaluation criteria.
