This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice.  This announcement constitutes the only solicitation for which proposals are being requested. This solicitation W91364-18-T-0005 is being issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-96. This solicitation is set-aside 100% for Small Businesses. The NAICS code that applies is 561621 and business size is $20.5 mil. This action will result in the award of a firm fixed price contract, utilizing simplified acquisition procedures under Full and Open competition, after exclusion of sources.

Services, non-personal: Requirement is for repair and modification services to Hangar foam suppression system equipment. See attached statement of work and additional documentation, for further details.

Also note the following: (1) previous requests for information/responses, and previous site visit minutes, are included in attachments, (2) any references to "Bldg 102" are to be disregarded--work is only related to Bldg 409, (3) current statement of work attached to this solicitation was revised from previous versions/prior to this solicitation.

All attendee names shall be forwarded to the Contracting Officer (Gordon Strom, [email protected] ) no later than 3:00pm, 21 Mar 18. Representatives shall report to front gate entrance, reside at parking location as determined by Security Forces, and will be escorted back to meeting by a Government representative. Note that failure to arrive at front gate prior to 1:55pm on day of site visit, may be grounds for exclusion from site visit, as there will a one-time escort for all attendees (i.e. escort will not return to front gate for those who arrive late).

Anti-Terrorism training requirements are attached to this solicitation.

Evaluation of offers: lowest priced technically acceptable (LPTA). Summary of how contractor will perform work shall be submitted, along with cost estimate. Contractor is not required to utilize attached contractor estimate form template; however, similar format and detail is expected to be submitted as part of offeror proposal. All bidders shall have current System for Award Management (SAM) registration, as a part of valid offer submission.

See attached Provisions and Clauses document, which applies to this solicitation. US Dept of Labor wage rate determination number 2015-4735, rev 5, dated 10 Jan 18, applies to services performed for this requirement. If wage determination is updated between time of solicitation and date of award, current wage determination will be included in contract.

Request for Information (RFI) template is attached to this solicitation; contractors shall utilize this document for all requests for information related to this requirement (regardless of type of inquiry).
