This announcement constitutes the only solicitation and a written solicitation will not be issued. Proposals are being requested for Request for Proposal (RFQ), VA243-15-Q-0434 for the following product: Inspection and Maintenance of Wastewater Treatment Facility Please see the Statement of Work at the end of this section for full details NY Harbor Healthcare System Brooklyn Campus, 800 Poly Place, Brooklyn NY 11209 who anticipates not conducting discussions with offerors submitting quotations under the subject solicitation and reserves the right to make contract award based on the initial offer submitted. Requirements may be added by future amendments to this solicitation. The Government contemplates award of a Firm Fixed Price contract resulting from this solicitation. SYNOPSIS/SOLICITATION Proposals must be received no later than 4:00 pm, EST on February 6, 2015. Offers may be submitted via email to Corey Swinton, [email protected]. When submitting offers, please note the solicitation number VA243-15-Q-0434 in subject line of the email. Please note that the offeror assumes all risk for any delay in the transmission of their proposals. Any offers received after the 4:00 PM EST deadline will not be considered. All offerors are encouraged to review FAR 52.212-1, INSTRUCTION TO OFFERORS-COMMERCIAL ITEMS and Internal Instruction, INT L2.11-4, E-MAIL QUOTATIONS and to submit their proposals early. SOW 630-15-1-6455-0013 1. Introduction and Overview Wastewater cleaning. 1.1 Background Mandatory cleaning and inspection of the facility wastewater system, 1.2 Scope of Work Inspection and maintenance of wastewater system.Cleaning and inspection of the acid neutralization system. Removal of sludge and waste in limestone tanks. Pipe and trap cleaning. Install limestone chips 1.3 Objectives To keep house pump free from debris 1.4 Period of Performance Five working days from date of order 2. References 3. Requirements 3.1 Tasks The contractor shall conduct the following tasks: Mobilization and de-mobilization Confined space entry permit and lock-out tag-out Odor control Removal of sludge and waste in limestone tanks Pipe and trap cleaning (including effluent piping) Inspection Install lime chips Install new gaskets and bolts to seal covers/testing Grab sample analyzed for toxicity characteristics Leachate Process (T.C.L.P.) 8 RCRA Metals 3.2 End Results/Deliverables 3.2.1 List of Deliverables by Task For large efforts, deliverables may be divided by subtask. The following table provides a complete listing of the required deliverables by task. The table includes, Task No. and Name, End Result/Deliverable, Tool for creating it, Acceptance Criteria, and Intended Use, as applicable. Task End Result/Deliverable Schedule/Milestone 3.3 Schedules/Milestones The contractor shall maintain a single project schedule from which various project reports shall be produced. The following reports shall be provided: 3.3.1 Who Does What When Report The "Who does what when" report shall be provided by the contractor with the initial submission, and again following negotiations. This report will be used by the Government to assess the adequacy of the resources proposed by the contractor to accomplish the SOW. 3.4 Other Considerations Include here any other relevant information for the performance of the SOW that does not fit elsewhere. 4. Progress/Compliance The Government requires the following from contractors in order to monitor progress and ensure compliance: * Weekly Status Report * Weekly Meetings * Monthly Progress Report * Project Management Team (PMT) Meetings * Program Reviews * Outlines and Drafts 5. Transmittal/Delivery/Accessibility The contractor shall provide [x] hard copies of each deliverable and one electronic version. 6. Notes If for reasons of clarity or brevity previous sections need further amplification, use this section for that purpose and for all other information that does not logically fit into previous sections. The information designed to assist in determining the applicability of the specification and the selection of appropriate type, grade, or class of the product or service.
