This solicitation, identified as AG-3395-S-13-0026, is issued as a Request for Quotations using simplified acquisition procedures. Issue date of this synopsis/solicitation is June 4, 2015. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 of the Federal Acquisition Regulations, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are requested and a written solicitation will not be issued.

VENDOR MUST HAVE AN ACTIVE REGISTRATION IN THE SYSTEM FOR AWARD MANAGEMENT (http://www.sam.gov) in order to receive an executed Blanket Purchase Agreement from this solicitation.
DESCRIPTION OF REQUIREMENT: Repair work on FSR 87 Ft. Seybert Road including road reconditioning, ditch restoration, and replacement of aggregate subgrade and surfacing from milepost 0.0 to milepost 7.0. Begins at milepost 0.0 on FS 87: 38 degrees 38'54.71"N, 79 degrees 2'44.46"W.
LOCATION AND DESCRIPTION OF WORK
Work will occur at the identified mile locations on FSR 87 Fort Seybert Road, on the North River Ranger District, Rockingham County, George Washington & Jefferson National Forests. Project is located approximately 5 miles west of Fulks Run, VA. Take exit 257 off of Interstate 81, staying on US RT 259 toward Timberville/Broadway. Continue on Rt 259 to Fulks Ru8n. Take a left onto SR 818, Little Dry River Rd. Continue to FSR 87 Ft. Seybert Road.
DELIVERY: Contract time will be 50 calendar days from the receipt of an awarded purchase order by the successful awardee. Delivery times shall be scheduled so as to arrive at placement locations between the hours of 8 a.m. and 5 p.m. Monday through Friday unless prior arrangements have been agreed upon. Placement of aggregate shall be as directed by the Forest Service on-site representative.
EQUIPMENT: Trucks used for hauling the aggregate material shall have a clean, tight bed, and shall have a minimum payload capacity of 15 tons.
INSPECTION: Inspection of equipment shall be allowed during all segments of the delivery process. Acceptance of the in-place aggregate shall be contingent upon approval by the Forest Service. Materials and in-place product shall comply with VDOT specifications.
MEASUREMENT: Certified weight tickets for the aggregate delivered and placed as directed will be the basis for payment.

• Representations and Certifications, attached with this notice.
• Experience and Past Performance Questionnaire, attached with this notice.
AWARD: Award will be made on basis of Best Value to the Government, price and other factors considered. Other factors include past performance and experience. Price is more important than non-price factors. The Government may choose to accept other than the lowest priced proposal when the perceived benefits of a higher priced proposal merit the additional cost. A fixed-price contract will be awarded from this solicitation.
