The Contractor shall provide one (1) Family Medicine Physician to provide services required for Government beneficiaries. The Contractor shall provide care at the 374th Medical Group, Yokota AB, Japan, also referred to as the "medical treatment facility" (MTF) herein. The Contractor care shall cover the range of services provided in a civilian medical treatment facility. Performance shall be according to the requirements contained in this performance based work statement (PWS), professional standards of the Joint Commission (JC), and Health Services Inspection (HSI). See the attached PWS for the detail.

Physician shall be a person who has American Nationality.  No green card holder is acceptable.
