Seeking sources to provide embalming chemicals and supplies.  The proposed contract action will be procured by blanket purchase agreement (BPA) and the method of payment shall be the Government Purchase Card (GPC).  The primary delivery address will be at the Air Force Mortuary Affairs Opperations Center at Dover AFB, DE.  There will be an additional delivery address to Kadina AFB, Okinawa, Japan.  The address for Kadina is as follows:
