Travis AFB is seeking sources to perform hospital laundry services for David Grant Medical Center, Travis AFB, CA. The Contractor shall provide all necessary labor, materials, and transportation to provide this service. Period of Performance will be 1 Oct 2012 through 30 September 2013 with four one-year option periods. Responses to this notice should include company name, address, point of contact, Cage Code, size of business pursuant to North American Industrial Classification System (NAICS) and must respond to the following questions:
1) Is your business a large or small business?
2) If small, does your firm qualify as a small, emergency business, or a small, disadvantaged business?
3) If disadvantaged, specify under which disadvantaged group and is your firm certified under Section 8(a) of the Small Business Act?
4) Is your firm a certified "hub zone" firm?

6) Is your firm a certified Service-Disabled Veteran Owned and Veteran-Owned?
7) Does your firm have an Environment Management System Certification (EMS)?
8) What qualifications/certifications does your firm have?
9) What is the average work experience of your personnel?
10) Does your firm perform background checks?
11) Does your firm have a quality assurance plan in place? What quality assurance methods and quality standards does your firm use?
12) Do you currently have a government contract?
13) Does your company launder and repair customer-owned goods ("COG")?
14) Does your company have a contingency plan in place in case of loss of utilities, medical emergencies, natural and/or man-made disasters,fire inclement weather, work stoppage, or major accidents?
15) Does your company launder and repair new linen and special handling articles?
16) If so, what is the turnaround time for new linen and special handling articles?
17) Does your company follow the Infection control guide lines as outlined and IAW California Administrative Code, Title 22, Social Security, Division 3, Licensing and Certification of Health Facilities & Referral Agencies and commercial laundry standards?
18) Does your company launder each facility's linen separately?
19) What is the turnaround time for the laundering of routine linen?
20) Has your facility been granted accreditation by the Joint Commision on Accreditation of Healthcare Organizations (JCAHO)?
21) Has your facility been granted accreditation by the Healthcare Laundry Accreditation Council (HLAC)?
22) What performance percentage thresholds are used most by your customers?
23) How are the thresholds measured?
24) What happens when the performance thresholds are not met?
25) What happens when the linen is returned in a soiled cart?
26) Do customers ever complain about too much lint?
27) What types of linen are handled (special) or differently and how is it handled differently?
Interested businesses are required to submit a Statement of Capabilities (SOC) demonstrating:
1) Ability to manage the services, description of past performance on previous
contracts, dollar value of each contract, number of employees for each, the types and
range of skills of employees for each, and point of contact and telephone number of the
contracting activity for each contract.
2) A capability statement, including summary of relevant performance history (with points of contact and applicable telephone numbers, FAX numbers, addresses and email addresses, if available) within the past three years.
This is not a request for proposals and in no way obligate the Government to award any contract. Responses must be received in writing no later than 12 Dec 2011. Firms responding to this announcement which fail to provide all of the required information cannot be used to help the Government make the acquisition decisions which is the intent of this synopsis.
