Treasure Island Job Corps Center / Adams and Associates, Inc., contracting for the U.S. Department of Labor for the operation of the Treasure Island Job Corps Center - a 532 student residential training facility located at 351 Avenue H, San Francisco CA 94130, is currently soliciting written bids for a Center Physician Services.

All quotes must be submitted in writing to the Purchasing Department. Quotes received after the deadline and/or without all requested documentation will not be considered.

SCOPE OF SERVICES

The Center Dentist will provide students of the Treasure Island Job Corps Center with the services stated below. Such services will be provided as authorized by the Center Director or his/her representative.

Dental services will be provided in the amount of 16 hours per week on Mondays and Tuesdays.

The Base Period of Performance will be November 1, 2015 and the end date July 31, 2016 and August 01, 2016 to July 31, 2017.

In addition to the above period of performance, additional pricing shall be provided for 3 option period of performance listed: August 1, 2017 through July 31, 2018, August 1, 2018 through July 2019 and August 1, 2019 through July 31, 2020.
