Mingo Job Corps Center, in Puxico, MO has a requirement for on-site Dental Services for a Base Year and four (4) Option Years to be exercised at the sole discretion of the Government. Wage Determination WD 2015-5101 Revision 15 applies to this acquisition and is incorporated as Attachment 1.

A single, firm-fixed price award is anticipated as a result of this Request for Quote. The Contractor shall be required to provide services as detailed in this solicitation.
