This is a request for proposal based on the attached Statement of Work (SOW) to provide Dental Services at USMMA.

The Dentist, Dental Assistant, and Dental Hygienist shall be NY State licensed, meeting all requirements of the NY State Board of Regents for the purposes of rendering on-site dental services to the Regiment of Midshipmen. Must be clinically experienced with a scope of practice which includes, but is not limited to patient age groups between 18 through adult.

Weekly work schedule is between 22 and 40 hours, depending on position; daily hourly schedule dependent on Academy needs.

This will be a one-year contract with 4 option years; total 5 years
