Work shall be performed at the Guthrie Job Corps Center; a Federally funded vocational/educational training facility operating year around.

The Center Dentist Hygienist is an independent contractor, and not an employee, representative, or agent of the Guthrie Job Corps Center.

The Center Dental Hygienist will provide services not to exceed thirty-two (32) hours per week, during a schedule determined by the Center Dental Hygienist and the Manager of the Wellness Center to meet the health requirements of the center enrollees.

Period of Performance for this agreement extends from July 1, 2010 through June 30, 2011.

QUALIFICATIONS:

Certifications, licensing or accreditation as required by the State of Oklahoma.

Experience in providing dental care for adolescents and young adults.

Provision of dental services to youths who are economically and socially disadvantaged.

Willingness and ability to carry out duties and responsibilities equal to those specified in the attached sub-contract.
