The Shreveport Job Corps Center is soliciting cost proposals for qualified professionals to provide subcontracted dental hygienist services to students (ages 16-24) for the following position: Dental Hygienist: Services are to be provided for a period of 15.0 hours per week, 48 weeks per year on a fixed price service basis. Contract will be awarded for the period of November 1, 2009 to August 31, 2011 with a potential for 3 pre-priced option years. In collaboration, coordination and consultation with the Center Dentist, the Dental Hygienist will perform services as outlined in the Job Corps Policy and Requirements Handbook (PRH) and the Oral Health and Wellness Technical Assistance Guide as permitted by Rules of the Louisiana Board of Dentistry, Rules Governing the Practice of Dental Hygienists. Proposal must contain copy of State of Louisiana license, resume, monthly/hourly rate. Job Corps is a national training and employment program administered by the Department of Labor to address the multiple barriers to employement faced by disadvantaged youth.
