1. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued.
2. AG-0202-S-11-0005 is issued as a request for quotation.
3. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition circular 05-43. The clauses 52.214-4 and 52.214-5 are incorporated by reference.

Illumina Custom, high-density genotyping assay which allows a large amount of data to be generated in a short amount of time. The assay requires reagents and materials designed and synthesized according to sequencing data. Sequencing data will be available upon award. Items are described as follows:
