The Village of Schaumburg is seeking proposals for medical services for the Village as well as the Board of Fire and Police Commission which shall include, but not be limited to, pre-employment physical examinations and/or substance screening, Fire Department Hazardous Materials (Hazmat) physical examinations, various examinations for employees that include fitness for duty examinations, post-accident examinations and testing, immunizations, annual respiratory questionnaire review, blood lead testing and audiograms, annual wellness programs including smoking cessation, non-DOT Drug, alcohol, nicotine testing, and Federal DOT CDL drug and alcohol testing.
