The Centers for Disease Control and Prevention (CDC) intends to award a sole source firm fixed price purchase order to Fortress Medical Systems, Inc., 32 10th Ave S. Ste 205, Hopkins MN, 55343-9481 for renewal and support of 16 Clindex Licenses. Fortress Medical Systems, Inc. is the sole provider for brand name Clindex Licenses.

This contract action is for supplies or services for which the Government intends to solicit and negotiate with only one source under the authority of FAR 13.106-1(b) and 10 U.S.C. 2304(c)(1). Interested persons may identify their interest and capability to respond to the requirement or submit proposals.

This notice of intent is not a request for competitive quotations; however, all quotations/responses received by December 17, 2014 at 10 a.m. Eastern Standard Time of the issuance of this notice will be considered by the government. A determination by the government not to compete this proposed contract based upon responses to this notice is solely within the discretion of the government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. TASKS TO BE PERFORMED

The vendor shall perform the following tasks:

A. Remotely renew the 16 current licenses for CDC Botswana upon receipt of the Purchase Order.

B. Provide technical support as needed throughout the 12 months of the license validity.

• Respond to all queries and requests for technical support by phone or email within one week of receiving the request.
• The vendor should be able to provide the following technical support:
i. Providing system related trouble shooting.
ii. Formulating complex SQL queries for retrieving data in Clindex.
iii. Explaining and advising on advanced features of Clindex to data management team to increase ability to retrieve data and reports.
iv. Providing on-going support on the customized off-line data entry feature.
