The Navajo Area Indian Health Services, Gallup Indian Medical Center, Purchasing Department is looking to procure for Carl Zeiss Cirrus 600 equipment and extended PM/warranty service contract for the Optometry Department at Tohatchi Healthcare Center, Tohatchi, New Mexico.
Place of Delivery: Tohatchi Healthcare Center, 07 Chooshgai Drive, Tohatchi, NM 87325. FOB shall be destination, if applicable
Period of Performance: Date of Award for one (1) Base Year w/two (2) Option Years
This is a brand name or equal solicitation. The equal equipment descriptions must include, in addition to the manufacturer's name, a general description of the salient characteristics (physical, functional, or performance) equal to item requested in order to be acceptable for award. Please review the attached Technical Data document for the required equipment specifications.
The government intends to award one firm fixed-price award to the offeror resulting from this RFQ to the responsible offeror whose conforming to the RFQ will be most advantageous to the Government, price and other factors considered. The following factors will be considered: 1) Delivery, 2) Warranty, 3) Special Features of the equipment required, 4) Technical Services availability and features.
There is no Defense Priorities and Allocations System (DPAS) rating assigned to this acquisition lead time.
