The Library intends to issue the requirement described above as a sole source order to The Brailler Depot in New Jersey. Any firm that wishes to be considered for this requirement must submit a capability statement that demonstrates its ability to meet the work described in the attached draft Statement of Work. The capability statement must not exceed five (5) pages double-spaced and requires the following information:

1) Company Name, DUNS Number, Address, Point of Contact, phone number and email address;
2) Corporate structure (corporation, LLC, sole proprietorship, partnership, limited liability partnership, professional corporation, etc.);
3) References, including name, phone number and email address of individuals who can verify the demonstrated capabilities identified in the responses;
4) Socioeconomic status, including whether the vendor is a woman owned, minority owned, blind or service disabled veteran owned; and
5) Resumes for key personnel that would work on the contract for individuals with knowledge and experience relevant to the requirement described in the draft Statement of Work (Resumes are not counted as part of the 5 page limit for capabilities statements).
