This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; Quotes are being requested and a written solicitation will not be issued.
Solicitation number is F5N0CC0069AG02. This solicitation is issued as a Request for Quote (RFQ).
This solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2020-05, DPN 20190430, AFAC 2019-1001 Revision 1.
This acquisition is conducted as a 100% small business set aside.
This requirement will be fulfilled using the Simplified Acquisition procedures, FAR Part 13.
The associated North American Industry Classification System (NAICS) code is 334510 Electromedical and Electrotherapeutic Apparatus Manufacturing.
Description of Requirement: The government anticipates awarding one single Firm Fixed Price contract. The contract consists of furnishing all labor, equipment, devices, tools, materials, transportation, supervision, permits, notifications, and all miscellaneous requirements to purchase Benson Medical Instruments (Brand name or Equal) Audiometers and acoustic simulator. Line Item 0001: 1 each Benson Medical Instruments-500303-001 CCA-200 mini system audiometer includes headset, response switch and cabling to connect to a PC. Brand name or Equal
Line Item 0002: 4 each Benson Medical Instruments-500303-005 CCA-200 mini expansion audiometer. Includes headset, response switch and cabling to connect to a PC. Brand name or Equal
Line Item 0003: 1 each Benson Medical Instruments-500132-001 1000 Bio-Acousti Simulator. Brand name or Equal
Salient features of the BAS-200:

Please ensure your offer is good through October 1, 2020. All quotes will be evaluated based on a LPTA (lowest price technically acceptable basis) and quotes should be sufficiently detailed in order to make that determination.
