This is a RFQ for the following equipment. All equipment is serviceable under MOA DHA-2017-R-402.

(7) of 500303- 001 Full system audiometer which includes: audiometer, CCA 200 Mini, software, headset, Response switch, USB CIC, power supply, power cable, 12 ft. serial extension cable, booth mounting plate, and mounting screws.

(12) of 500132-001- BAS 200 Bio Acoustic Simulator to include: mounting bracket, simulator, and 1-3.5 in. patch cord and mounting screws.

(6) of 500303-005- CCA 200mini Expansion Audiometer to include: audiometer, headset, response switch, 6 ft. serial extension cable, booth mounting plate, and screws.

(15) of 300394-001- individual 3.5 in. patch cables.

All equipment must work with the DOEHRS HC software and DOEHRS DR website. Equipment will be serviced and maintained in accordance with the Memorandum of Agreement number: DHA-2017-R-402 by Navy and Marine Corps Public Health Center (NMCPHC). Under this agreement equipment will be serviced and delivered throughout the calendar year based on calibration schedule to ensure that there isn't any down time. NMCPHC will provide and fund maintenance at the NMCPHC Calibration Laboratory and/or send to vendor.
