CITY OF SANDY BELL AND 362ND AVENUE Bids due at 2:00 PM, April 12, 2022 INVITATION TO BID Sealed bids for the construction of the Bell and 362nd Avenue project will be received by the City of Sandy until 2:00 PM, Tuesday, April 12, 2022. Pursuant to ORS 279.370 (2), within two (2) working hours of the Bid Closing all Bidders shall submit to the City a disclosure form as furnished with these bid documents identifying any first-tier subcontractors (those entities that would be contracting directly with the prime contractor) that will be furnishing labor or materials on the Contract, if awarded, whose subcontract value would be equal to or greater than: • Five percent of the total Contract Price, but at least $15,000 or • $350,000 regardless of the percentage of the total Contract Price. Bids and Disclosure forms should be addressed to Ms. Jennifer Coker, P.E, Public Works Director, 39250 Pioneer Blvd., Sandy, OR 97055. At 2:15 PM, all bids will be opened and read aloud in the Council Chamber of the City Hall, Sandy, Oregon. Only bids for which a disclosure form has been submitted within two (2) working hours of the Bid Closing date and time will be considered responsive. The project will construct a new segment of SE 362nd Avenue north of Highway 26 approximately 1500 feet and construct a new segment of Bell Avenue from SE 362nd east to the existing Bell Avenue dead end approximately 3,200 feet in length. Improvements include new street construction including new curbs, concrete sidewalks and driveways, ADA ramps, retaining walls, arch culvert, storm drainage facilities, landscaping, street lighting, water and sanitary improvements, new signal and roadway widening at Highway 26. Any addendums to the Bid Docu ments will be posted at the link shown in the Addendum folder. Bidders must be pre-qualified to bid on this project. Bidders not currently prequalified by the ENGINEER or the Oregon Department of Transportation (ODOT) must submit a correctly completed pre-qualification form no later than seven (7) days prior to the bid opening on forms available from the Oregon Department of Transportation. Forms are available at ODOT's website: http://www.oregon.gov/ODOT/CS/OPO/ construction/prequalification.shtml No proposal shall be considered or received unless the bidder is registered with the Construction Contractor's Board as required by ORS 701.021. To be considered, proposals must be completed on the forms provided, in the manner prescribed in the bidding docu ments, submitted intact, and accompanied by a 10% bid security in favor of the City of Sandy. Each bidder must indicate in the space provided on the Bid Form whether they are a resident or non-resident bidder pursuant to ORS 279.029A.120. This public works project is subject to Prevailing Wages. By submission of a bid, each bidder certifies that the provisions of ORS 279C.800 to 279C.875 and Oregon Prevailing Wage Rates are included in the bid prices. Contractors need not be licensed for asbestos handling per ORS 468A.720 to complete this project. A pre-bid will not be held for this project. The contractor submitting the successful proposal will be expected to furnish a suitable performance bond from a surety company registered to do business in Oregon and comply with applicable insurance requirements. The City of Sandy reserves the right to reject any or all bids not in compliance with all prescribed public bidding proce dures and requirements, may reject for good cause any and all bids upon a finding of the agency that it is in the public interest to do so and may waive all informalities.
