The National Park Service is required by Department of the Interior policy to investigate all serious accidents including employee fatalities. A key element of the policy requires that a Serious Accident Investigation Team (SAIT) is constituted to determine the facts and the sequence of events related to the incident; determine contributing causes of the incident including management system deficiencies, when applicable; to analyze investigative information to determine the root causes for any unsafe acts, conditions or behaviors and management system failures; and to make recommendations of corrective actions to prevent recurrence. The National Park Service is interested in contracting for delivery of professional facilitation of root cause analysis to assist a Serious Accident Investigation Team (SAIT) in developing cause and effect charting and potential corrective actions of an incident that involved an employee fatality.
