This amendment is being issued pursuant to FAR 52.212-4 Contract Terms and Conditions-Commercial Items. The purpose of this modification is to 1) extend the response date to April 3rd, 2017 at 9:00 am 2) to request a firm fixed price to include services, materials, and personnel as outlined in the original Request for Quote 3)to request a DUNS number of the contractor to receive award and 4) to request a Memorandum of Understanding(MOU) or Cooperative Research and Development Agreement (CRADA) between the Institution containing the lab equipment and the UNITED STATES ENVIRONMENTAL PROTECTION AGENCY if the proposing contractor does not own the equipment to be used under the proposed contract. No grant information shall be submitted with the quote as this Request for Quote is for a contract and not a grant. No F&A or indirect rates shall be included in the firm fixed price quote.
