FAR, Subpart 5.204-NOTICE OF INTENT TO SOLE SOURCE

The Centers for Disease Control and Prevention (CDC), National Center for Emerging and Zoonotic Infectious Diseases (NCEZID) intends to solicit a single source and award a firm-fixed-price contract to Affymetrix, Inc. located at 3420 Central Expressway Santa Clara, CA, 95051-0704. The purpose of this procurement is to provide yearly maintenance service for the following instruments:

This equipment is propietary to Affymetrix, Inc. and there are no other suppliers of these components or have a similar type of lithography platform. Furthermore, there are no other vendors that could provide maintenance and repair of this propietary equipment in which Affymetrix would consider compliant to the equipment warranty.

Only one responsible source and no other supplies or services will satisfy agency requirements. The associated NAICS Code is 334516. The Classification Code is 6640 for Laboratory Equipment and Supplies.

This is not a solicitation and proposals are not requested. Interested organizations may submit their capabilities/qualifications statement to perform the effort, in writing to Mrs. Nancy Khalil, via email at [email protected] with "Solicitation Number 2016-Q-65742" referenced in the subject line, no later than 11:00am EST on Thursday, August 4, 2016.
