This is a combined Synopsis/Solicitation for commercial items prepared IAW the format in Federal Acquisition Regulation (FAR) subpart 12.6,  as supplemented with additional information included in this notice.  This announcement constitutes the only solicitation;  Proposals are being requested and a written solicitation will not be issued. 

The solicitation number for this procurement is F3QCCM0245A002 and is issued as Request for Quotation.   This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-41, dated 13 Apr 2010 and Class Deviatioin 2009-0005 and Defense Acquisition Circular (DAC) 91-13 Correction.  The North American Industry Classification System (NAICS) is 334510.

 Description:  The purchase of 17 each, Philips HeartStart FR2+ AED Public Access Defibrillation Units.

 Price(s) should be FOB Destination (Robins AFB GA 31098).   Request delivery date is 30 days ARO or earlier.

 Brand Name Only:  Philips HeartStart FR2+ AED and/or distributors

 Justification for Brand Name Only:  By order of the AFMC Command Instruction 10-301, 08 Apr 2008, Item #3 AED Model Selection which states:  Model selection will be determined by the installation Medical Director, (A single AED model should be purchased for the installation).  This selection has already been determined and other Groups on base have purchased this model.  This fact determines that we must purchase the "Philips HeartStart FR2+ AED".

 Provision at FAR 52.212-1, Instructions to Offerors Commercial Items applies to this acquisition.  The following addenda is provided to this provision.  Quotes should be submitted including the following information:  solicitation number, name, address, telephone number of offeror, price and any applicable discount and payment terms.  Signed quote must indicate quantity, unit price and total amount.  Offerors that fail to furnish the required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration.   In accordance with FAR 52.212-2, Evalutation-Commercial Items.  The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and price related factors considered.  All offerors must have FAR 52.212-3, Offeror Representations and Certification - Commercial Items completed on line annually.  If not, on line certification can be completed at http://orca.bpn.gov.  Offeror must also be registered in Central Contractor Registration (CCR).  Information concerning CCR registration requirements may be viewed via the internet at http://www.ccr.gov/or by calling 1-888-227-2423.  All FAR clauses may be viewed in full text by viewing the Federal Acquisition Regulation Table of Contents via the internet at http://farsite.hill.af.mil.
